Template: Record of Review of Assessment Evidence and Assessor Decisions

This tool is suitable to results of analyses on assessment decisions made.
 
Outcome Standard 1.5.2a requires a process that includes people with specified credentials to review the judgements made on student evidence by people with the specific qualifications and experience to allow them to make decisions about competency.
 
This template is designed to be used as part of the validation of assessment judgements process to document:
The evidence under review – including how it was selected for review
Confirmation of the details of the assessor/s who made the assessment judgements
The outcomes of review of the marked assessment evidence
 
 
Note that this template on its own does not fulfil or support the entire process that would occur to demonstrate meeting Outcome Standard 1.5.2a as it is not an entire framework.
